I'd call it a fantasy.

    What I’m getting at is that the piñata/unicorn is meant to represent that sense of self actualization. Knowing what you are and being proud, or at least not ashamed of it. The imagery of the unicorn/piñata’s come up 4 times now. At the beginning of the comic, in hell(?), with the psycho lord, and right now. Each appearance has dr knot either trying to avoid it, or get closer to it. Like he would LIKE to be proud of what he is, but his upbringing and the societal norms around him feel ashamed and avoidant. Really, the only thing keeping him from that piñata is himself.
